What is the purpose of NG tubes
Decompression
Nutrition
Lavage – large bore tube to clean stomach, intoxication, overdose.
Diagnostic – view anatomy of stomach
Explain the principles underlying nasogastric/nasoenteric intubation and removal
Do not insert NG if -Unconscious No gag or swallow reflex Neck or facial trauma or surgery Basal skull fracture Esophageal varices or obstruction Recent upper GI surgery Thermal or chemical injury to upper GI or Respiratory tract
REMOVAL -Assess patient Prepare the patient Gather equipment Position patient Put gloves on Disconnect suction Remove anchoring tape Clear NG tube and remove Provide mouth care, nose care and give sips of water Measure drainage Dispose off equipment Document
Describe the observation to make before, during, and after NG intubation and removal
-Check the physician’s order
Assess the patient - nares
Prepare patient – the procedure
Gather equipment
Wash hands
Provide privacy to the patient
Position self – 30 degree
Examine nose
Measure and mark (nose, ears, xyphoid)
Lubricate the tube
Alert the client about the commencement of the procedure
Instruct client to extend neck back against pillow
Insert the tube slowly through the naris, with curved end pointing downward
Once past the nasopharynx, instruct client to flex his head forward and swallow water
Advance 2 to 5 cm with each swallow
Continue to advance the tube until desired length has been inserted
Secure the tube slightly to nose, while checking for placement
Once placement confirmed, anchor the tube
HOB should remain elevated at 30 degrees
Remove gloves and wash hands

Irrigation of NG TUBE
Purpose: patency of tube
S & S of occlusion: decreased drainage, N&V, gastric distention
Clean procedure
Instill 30-50 ml saline gently and aspirate
Keep equipment at bedside and change Q24 hrs
Document
Checking for placement of tube
Ask client to talk
Inspect posterior nares for evidence of coiled tube
Check aspirate for pH value
