Painful third nerve palsy

Distal sensory loss, tingling + absent ankle jerks/extensor plantars + gait abnormalities/Romberg’s positive →
subacute combined degeneration of the spinal cord
Patients often present with reduces Glasgow coma score, quadriplegia, miosis, and absent horizontal eye movements.

Brain abscess:
IV 3rd-generation cephalosporin + metronidazole
Middle-aged, personality changes, involuntary movements
?Huntington’s disease
fatigable, painless muscle weakness that improved with rest
myasthenia gravis
Contralateral homonymous hemianopia with macular sparing and visual agnosia
posterior cerebral artery
presents with lower limb stiffness and imbalance
degenerative cervical myelopathy [DCM]
suspected encephalitis
Start aciclovir in all cases of suspected encephalitis
Urinary incontinence + gait abnormality + dementia
normal pressure hydrocephalus
first line radiological investigation for suspected stroke
Non-contrast CT head scan
