Anterior cerebral artery stroke
COntral leg weakness, numbness with higher function defeceit
Middle cerebral artery stroke
Covers most, contralateral hemiplegia, numbeessn including face + arms. Speech and homonymous hemaniopia
Posterior cerebral artery stroke
Loss of pain and temp sensation contralateral body and ipsi fave. Ataxia, nystagmus, vertigo. Lateral medullary syndrome, Wallenberg’s syndrome
Basilar artery stroke
Corticospinal, spinothalamic, & medial lemniscus. Locked-in syndrome.
Stroke Mx (ischaemic)
300mg aspirin
Thrombolysis (alteplase) if within 4.5 hours
Thrombectomy, depends on location. Up to 24 hours
Stroke Mx (haemorrhagic)
Nimidopine get BP <150
+neurosurgery opinion
TIA Mx
300mg aspirin
If >1 TIA in week admit
TIA within 7 d seen by specialist <24hrs
TIA over 7 d seen within a week
Diffusion weighted MRI & Carotid doppler
Migraine Mx
Prevent - Topimerate (propanolol for women of CBA)
Acute - Triptans (sumitriptan)
Cluster headaches Mx
100% oxygen + subcut triptans
Verapamil as prophylaxis
CNIII lesion
Ptosis
Fixed dilation
Eye down and out
CNIV lesion
Diplopia
Tilted head
Cant look down and in
