Mumps incubation & infectious period
12-25 days
2 days before to 5 days after parotitis onset
Mumps receptor and tropism
Receptor - glycans on cell walls
Tropism - T cells
Mumps clinical picture
Fever, IFI
Salivary gland swelling within 48 hours
Complications:
Orchitis - 5 to 10 days post parotitis
CNS - meningitis, encephalitis, deafness
Myocarditis
Mumps diagnostics
RT-PCR on buccal swab/seeum
IgM develops 5 days after symptom onset & remains positive for up to 4 weeks
Mumps PEP & IPC
No HNIG/MMR recommended by UKHSA
AMERICAN ACIP recommends a third dose when previously had 2 doses MMR
Exclude non-immune contacts from work/school between D12 to D25 post contact.
Mumps R0
10-12 in an unvaccinated population- much lower now with vaccine coverage
