If we are thinking about people with chronic disease states they have medications for us to review with them. We are thinking about people who have several medications and people with several health conditions so we are not necessarily talking about somebody that has a past medical history,significant for depression where they take one medication and they are controlled perhaps they might not benefit from medication therapy review as much as somebody who is taking warfarin to decrease their risk for stroke whose also taking lisinopril for their blood pressure management whose also taking atorvastatin to control or to help lower their cholesterol levels who is also taking etc so the pt has multiple medications and multiple disease states perhaps they have questions or problems with their medications perhaps they are taking medication like we talked about warfarin that requires close monitoring…who else might benefit? Those patients that have been ____________ so they are going from their home or where they have been living to then a hospital setting where medications might change…maybe a new disease state has been added on to their list and now they have to transition and we can help them as healthcare providers as pharmacists to go through everything with them and help them to organize and understand better what has been going on…perhaps those who obtain their medication from more than one __________ we know that due to prices…due to convenience we are not in the age where people are going to one pharmacy and getting all of their medications there instead rightfully so people are shopping around or insurance companies are forcing them to go into a mail order for one medication because of the price and then there required to get a 3 month supply from the CVS down the road or maybe then they can only get their antibiotics from the walgreens that is right next door to where they work there’s all sorts of possibilities and all sorts of reasons but we have to be open to see this happening and it be being something that we see more regularly now and could make things more confusing for them and also more difficult for us as pharmacist as we are trying to piece together the puzzle to help them have the best outcomes

Its been more difficult to have a close relationship because People pushed or forced to get their medication in certain places they are not necessarily coming to one pharmacy all the time to get every single prescription we are also incredibly busy…we have staffing issues we don’t have the time necessarily to always make a relationship and build it but MTM allows a little bit of more downtime so that focus one on one time with your patient that then really works to enhance that relationship
