What are theclinical features of MS?
Pyramidal dysfunction Optic neuritis Odd sensation Lower urinary tract dysfunction Cerebellar dysfunction Cognitive impairment
What symptoms would encompass pyramidal dysfunction?
Increased tone
Spasticity
Weakness
(Extensors of lower limb flexors of upper limb)
How does optic neuritis present?
Painful visual loss
1-2 weeks
RAPD
What are the symptoms of cerebellar dysfunction?
Ataxia
Intention tremor
Past pointing
Pendular reflexes
How is MS diagnosed?
At least 2 episodes suggestive of demyelination
Dissemination in time and place
McDonald criteria
How is MS investigated?
MRI
CSF
How are acute exacerbations of MS treated?
Oral steroids (if severe IV)
How can lower urinary tract dysfunction in MS be treated?
Bladder training
Anti-cholinergics (e.g. oxybutynin)
Desmopressin
Catheterisation
How can spasticity in MS be treated?
Baclophen
Botulinin toxin
What is the first line disease modifying therapy in MS?
Interferon Beta or Copaxone or Monoclonal antibody
