What are the differential diagnoses for sudden onset headache?
SAH
Migraine
Benign coital cephalgia (sex)
What investigations should be used to investigate SAH?
CT scan
LP
Cerebral angiography
What are the potential complications of SAH?
Re-bleeding Delayed ischaemic deficit Hydrocephalus Hyponatraemia Seizures
How might hydrocephalus present post SAH?
Increasing headache or transient altered conscious level
What are 50% of intracerebral haemorrhages secondary to?
Hypertension
What aneurysms arise on small perforating arteries in hypertensive ICH?
Charcot-Bouchard
How does ICH present?
Headache
Focal neurological defect
Decreased conscious level (if big enough to cause raised ICP)
How should ICH be investigated?
CT scan
Angiography (if suspected underlying vascular anomaly)
How is ICH treated?
Surgical evacuation of haematoma +/- treatment of underlying abnormality
