Motor Organization & Regulation Subsystems of an infant in NICU Flashcards
State Organization (subsystem)
- Deep sleep
- Active sleep/light sleep
- drowsy
- quick awake/alert
- active alert
- crying
Deep sleep
eyes closed, regular breathing, no spontaneous activity
active sleep/light sleep
sleep with eyes closed, sometimes rapid eye movements
with eyes closed
drowsy
eyelids fluttering, variable activity level with occasional mild startle, eyes may
be open by heavy lidded or closed
quiet awake/alert
bright look, focused attention on stimulation, minimal motor activity
active alert
infants eyes are usually open
crying
intense, loud, rhythmic & sustained crying that is difficult to stop with stimulation
Postural reflex & motor organization
motoric subsystem
Motoric subsytem
- involves tone, reflexes, posture & movement quality
- arching as response to stimulation
Motor Organization
Flexion & Extension of extremities during diaper change, heel lance or suctioning
Motor organization: hand movements
- backwards ext of fingers
- usually seen with painful procedures such as heel lance or endotracheal suctioning
- may be seen during non painful but stressful situations (bathing, chest pt, diapering)
Motor organization: Facial reactivity
*Seen with painful stimulations
*scales to determine if facial reaction is due to pain/non pain stimulation
Self Regulation (subsystem)
- state modulation
- attentional regulation
- positioning strategies
state modulation
infant’s ability to modify behavioral internal & external stimulation
Attentional regulation
● Clear, rhythmical and robust sleep states or crying
● Good self quieting and consolability
● Focused, bright eyes awareness with animated facial expressions such as
frowning, cheek softening, mouth pursing, cooing, attentional smiling
