UMN lesion
Cerebral hemispheres, cerebellum, brainstem, spinal cord
LMN lesion
Anterior horn cell, motor nerve roots, peripheral motor nerves

UMN tone
increased spasticity or rigidity +/- ankle clonus
hyperreflexia
LMN tone
Decreased (hypotonia, flaccid) or normal
UMN power
Reduced with pyramidal pattern of weakness (extensors weaker than flexors in arms, and vice versa in legs)
LMN power
Reduced in distribution of affected motor root/nerve
UMN reflexes
Exaggerated or brisk (hyperreflexia)
LMN reflexes
Reduced or absent (hyporeflexia or areflexia)
UMN plantar reflex
Upgoing/extensor (Babinski positive)
LMN plantar reflex
Normal (downgoing/flexor) or no movement
