dementia
umbrella term for a variety of diseases that cause irreversible changes in the brain. Symptoms: loss of memory, judgement, and reasoning and changes in mood, behaviour and communication abilities. Decline in ADLs
delirium
acute confusional state, is a potentially reversible cognitive impairment that often has physiological case.
depression
reduces happiness and well-being, contributes to physical and social limitations, complicates the treatment of concomitant medical conditions and increases the risk of suicide
Restorative nursing interventions
Whether accomplished in an older persons home or long term care institutions, stabilize chronic conditions, promote health and promote independence in basic and instrumental ADLs
Goals of end of life care
To learn to live fully with an incurable disease, with the help of a nurse. Different for each individual depending on personal beliefs (spirituality,religion,death its self).
Advance care planning (ACP)
Multidimensional process that involves health care providers in discussion with patients to ensure that they clearly understand their illness, it’s trajectory and available treatment options
