5 Compnents of a Well-Structured Form In Depth
Form Headline
-your form headline should introduce “What” of your offer. What will I be getting if I interact with your form?
Subhead
-The subhead can be used to add a clarifying description of what the offer is, this is the first part of “Why” I should care about your offer.
The Form
-This one’s easy. Your form goes here now that it’s purpose has been introduced.
CTA
-Your all-important call to action that describes what will happen when you click, or reinforces what it is you’ll be getting.
The Closer
-A closer is intended to add a last minute nudge to encourage a click. It can be social proof, urgency, a trust statement, or important details such as “Free” or “No credit card required” etc.

What is Form-First Design?
ensure that it has a solid context
Field Labels
influence the behavior of the audience by being clear about your labels
What are the 3 things you SHOULD NOT do when it comes to creating forms?
- Do NOT help people to leave your page!
- Do NOT provide an option for people to delete the content of the form with “cancel” or “clear form”.
- Do NOT show an extra confirmation page which is redundant
What is the Continuance Flow?
- Confirm the action.
- Ask for a second action, eg: up-sell/cross-sell.
- Clarify if it’s a download link OR check the email with the download link inside.
The Matt Sweezey Approach
- Confirm the action that just happened.
- Information hierarchy: telling your story in the correct order.
The “Thank You” or Confirmation Page
- Always ask for a second action for an opportunity for a cross sell, upsell, social share or answer a question.
- Do NOT ask for something that requires “too much” commitment/action steps
