What causes meliodosis and where is it found?
Burkholderia pseudomallei
Northern Australia, SE Asia especially Thailand
What does burkholderia pseudomallei look like?
G-ve bacilli with bipolar staining
How is meliodosis transmitted and how long is its latency period?
Contamination of cut with water
Inhalation of aerosolised water
Aspiration of water
1-21 days
What are the risk factors for meliodosis?
Diabetes
Alcohol
Immunosuppression
Renal disease
What clinical syndromes can you get in meliodosis?
Asymptomatic- common Neuro Parotitis - 1/3 kids Pneumonia- 50% Prostate abscess Osteomyelitis Septicaemia Derm- eryhematous lesion
How do Ohio diagnose meliodosis?
Blood cultures
Serology but will be positive if from endemic country
How do you treat meliodosis?
Very resistant
Meropenem or ceftazidime for 2weeks
Daily doxy and cotrimox for 3 months to eradicate
