What are the characteristics of vibrio cholerae?
G-ve comma shaped
Motile
Aerobic
Multiple serotypes with different case:carriers ratios
How is cholera spread?
Transmitted by water contaminated with faeces
What is the pathophysiology of cholera?
Toxin produced which produces cAMP in gut mucosa and leads to Cl and H2O being effluxed into lumen
What are the symptoms of cholera?
Profuse rice water diarrhoea
80% vomiting
75% have no symptoms
If severe D+V can lead to hypovolaemic shock
How do you manage cholera?
Volume replacement- match the output with salty sugar water
Zinc for children
Doxycycline prevents further spread but no effect on toxin
How do you prevent cholera?
WASH
Vaccine: oral live attenuated, 2doses, lasts 2yrs and can be used as PEP
