Aspirin MOA
Inhibits COX enzyme (TXA2) that promotes platelet aggregation
Aspirin Therapeutic Use
Ischemic strokes, TIAs, Stable/unstable angina, Acute MI, Previous MI, prevention of MI.
Clopidogrel MOA
blocks ADP receptors on the platelet surface - no ADP - no platelet aggregation
Clopidogrel Therapeutic use
Thrombotic events
Clopidogrel Considerations
Needs about ~5 days off prior to surgery to wear off in body
GPIIB/IIIA R Antagonist - abciximab
IV ONLY
“Super aspirins”
Used for acute coronary syndromes
Thrombolytics/Fibrinolytics - Transplasminogen alteplase MOA + Use
Removes already formed clot - given IV
Used for Stroke, MI, MASSIVE PE
Thrombolytics/Fibrinolytics - Transplasminogen alteplase - Contraindictions
MANY - patient specific - VERY cautious
Recent internal bleeding of ANY kind, known IC issues, active internal bleeding
