Heart problems - such as previous heart attack or angina
Breathing problems - asthma , COPD
Stomach or bowel problems - crohns , acid reflux
hormone imbalances - diabetes , thyroid dysfunction
(neurological) - fits/ faints/ seizures
conditions affecting bone/ muscle - arthritis , osteoporosis
Blood disorders - haemophilia , anaemia
Any other conditions I should be aware of ?
What should be covered in dental history
are they a regular attender
how often do they brush their teeth
any phobias
any problems with treatment/ LA in the past
what should be covered in social history
smoking - have you ever thought about quitting?
