Monoamine oxidase inhibitors
Therapeutic class: antidepressant

phenelzine (Nardil)
tranylcypromine (Parnate)
isocaboxazid (Marplan)
Mechanism of Action
to decrease effectiveness of monoamine oxidase
produces irreversible inhibition of MAO which intensifies effects of norepinephrine
Actions and Uses
depression, panic disorder
anxiety (off label)
Adverse Effects
orthostatic hypotension
diarrhea
headache
insomnia
abrupt discontinuation may cause rebound hypertension
overdose: seizures, respiratory depression, circulatory collapse, coma
Black Box Warning
Increase risk of suicidal thinking, not approved in peds
Contraindication
CV or cerebrovascular disease, hepatic/renal impairment, pheochromocytoma
Interactions
SSRIs (seretonin syndrome)
Avoid foods containing tyramine (hypertensive crisis)
Caffeine may cause cardiac dysrhythmias and hypertension
Herbal: ginseng (HA), ma huang, ephedra and St. John;s Wort (HTN crisis)
