Managing Neurologic Disorders in SR Flashcards
What are some common presenting complaints for a neurologic SR?
Cant stand or move
cant see
acting neuro
tongue out and cant chew or eat
stumbling, walking weird or ataxic
ADR
What are your top differentials for neurologic disease in a SR?
Listeriosis
Polioencephalomalacia
Paralaphostrongylus tenuis
Trauma
Rabies
CAE/OPP
Tetanus
Meningitis
Colostriudum perfringens
Lead
Water and salt toxicity
EEE/WNV
What are some common syndromes mistaken for neuro disease?
Lameness
Weakness
Ocular Disease
What are the most common goat brain lesions?
Listeria
Polio
What are some good history questions for a neurologic SR?
When did you last see him normal?
Gotten better or worse?
Eat or drink?
Any others showing symptoms?
Diet?
What have you treated with?
What are some important things to do on your neuro exam of SR?
PPE
Observe Distance: Mentation, gait, enviroment
Fever?
Examine Feet?
Hand on everything head to toe
What are important parts of the neuro exam?
Facial symmetry
Menace
PLRs
Tongue control
Gag reflex
Deep Pain
Spinal Reflex
What are some diagnostics that are useful for antemortem diagnosis of neuro disease?
CBC/Chem
Rads
CSF
CT
MRI
Ultrasound
How do you prevent the following:
Listeria
Polio
Meningeal Worm
Listeria - hard to prevent, feeding silage that is not rotten
Polio - prevent acidosis, consider sulfur source
Meningial worm - injectable ivermecti, concern GI resistance, prevent deer
What part of the CNS does Listeria effect and what signs will you see?
Brainstem/Cranial Nerve Disease
-Abnormal mentation, unilateral deficit, head tilt, circling, facial nerve deficit
What part of the CNS does polio effect?
Cerebral Disease
-Abnormal mentation
-Central blindness
-Opisthotonus
What part of the CNS does meningeal typically affect and what are the signs?
Spinal Cord
-Seasonal
-Normal mentation
-Willing to eat and drink
-Gait deficit, ataxia, cant stand
What is the prognosis for the following?
Listeria
Polio
meningial Worm
General neuro disease
Listeria - Good if treated early, poor if recumbent or severe
Polio- response to treatment critical
Meningial Worm - depend on degree CNS, long time to heal, nerve damage may be permanant
General neuro disease - poor if down and not eating, better up and responsive
How do you treat listeria?
Antiobiotics: Oxytetracying, PPG or Florfenicol
Anti-inflammatory - flunixin
Supportive
How do you treat polio?
Thiamine
mannitol and dexamethasone
What is the dose of thiamine to treat polio?
10mg/kg IM q 6 hours for days, taper
How do you treat meningeal worm?
Fenbendazole
Ivermectin
Flunixin
Dexamethasone
What kind of supportive care can help in neuro cases?
Orogastric feeding tube, IV fluids, thick bedding and repositioning
What are some post mortem diagnostics for these neuro animals?
Rabies - necropsy
Necropsy
Fixed brain helpful non rabies
What is key to remember when communicating to owners about neuro disease?
Warn that we may only get one chance to treat
Poor prognosis
Never get a confirmed diagnosis when treating these
