When should the London Underground Senior Operating Officer consider waivers?
When waivers will resolve the incident safely and more quickly. The LU SOO must base waivers on the rule book on a recorded real time risk assessment.
What are the incident categories, and who can declare them?
Cat 1 - Formal Incident management, declared by the Senior Operations Officer
Cat 2 - Local incident Management, declared by a Service Manager.
Cat 3 - incidents that require monitoring declared by a Service Manager
What are Cat 1 incidents?
Trains stalled for more than 30 mins
Full or partial line suspension
Serious infrastructure damage
Serious injury or loss
Multiple lines affected
You must declare a cat 1 incident for
A radio system failure
A major power failure
A major flooding incident
A confirmed terrorist attack
A major fire
A code red to all trains on a line
An evacuation of the underground
A person under a train
What are cat 2 incidents
An incident limited to one location or line
Trains stalled in tunnels for less than 30 mins
A radio cell failure
Temp local loss of traction current
Trackside fires
Trespassers
Person Ill on a train
Security incidents partial closure of cat A station
What are cat 3 incidents?
Escalator closures
Full or partial closure of a Cat A station not security related
Station control measures
When managing a cat 2 incident what should a Service Manager consider?
Continuously review the incident
Update the Duty Network operators manager with actions taken by whom and when
Keep the SOO updated of the situation
Confirm which radio talk group will be used
Actively monitor incident status and affect on train positions and on passengers
Request additional staff and resources if necessary
Get authority from a SOO before in enacting any waiver.
