Management of Chronic Pain Flashcards

1
Q

What are the two ways peripheral tissue injury changes the nervous system?

A

Peripheral sensitisation = reduction in threshold of peripheral afferent nociceptors
Central sensitisation = increased excitability of spinal neurons, rewiring in spinal cord, changes in brain

Q

What is the management of neuropathic pain?

A

Most often tricyclics (amitriptyline) and anticonvulsants (gabapentin)
Others = opioids, NMDA antagonists, sodium channel blockers, capaicin, GABA agonists

Q

How useful are using opiates to treat non-malignant pain?

A

Effective for pain = some pain may not be opioid sensitive though
Useful in nociceptive pain, but not really neuropathic pain

17
Q

What are some side effects of opiates?

A

Hormonal effects, immunomodulation, hyperalgesia
