What are some examples of functional symptoms?
Headaches, seizures, weakness, visual symptoms and memory loss
What are some other names for functional attacks?
Dissociated seizures, non-epileptic attack disorder, psychogenic non-epileptic attacks
What are some characteristics of functional attacks?
Not consciously mediated and patient has no control
Related to stress or traumatic events
Never benign
How are functional attacks diagnosed?
History, linguistic analysis, outpatient EEG and video with provocation, longterm video EEG monitoring
What causes a seizure?
Abnormal electrical discharge in part of brain = seizure mimics area of brain function
What are auras?
Epileptic seizures that occur in a part of the brain that controls sensation
What are some features of somatosensory auras?
Usually originate in somatosensory cortex = numbness, tingling, thermal sensations
What are some features of visual auras?
Due to occipital lobe issues = simple shapes, static or moving lights, flashing
What are some locations where pathology may cause auras?
Auditory aura = auditory cortex
Autonomic aura = temporal lobe
Vertiginous auras = temporal-parietal lobe near visual and auditory association areas
What are some features of functional attacks?
Prominent motor activity
Episodes of collapse with no movement
Prolonged duration = 10-20mins
What are some signs of an abreactive attack?
Fear, gasping and hyperventilation
What is the treatment for functional attacks?
Withdraw any anti-epileptic drugs
Positive support and counselling
Treat associated anxiety and depression
