What is the mechanism of action of the macrolides?
Binding to the ribosomes of susceptible microorganisms, inhibiting protein synthesis.
For what indications may the macrolides be used?
RTIs, Lyme disease, pertussis.
What group of patients may particularly benefit from macrolide use?
Patients who are allergic to penicillins.
What are some side effects of the macrolides?
GI side effects, hepatotoxicity, rash.
Compare the GI side effects of erythromycin and azithromycin & clarithromycin.
Mild and less frequent with azithromycin and clindamycin.
What are some cautions for the use of macrolides?
Caution in patients who are predisposed to QT interval prolongation - electrolyte disturbances, certain drugs. May aggravate myasthenia gravis.
