Lymphedema Management

Phase 1

A
  1. short stretch compression bandages
  2. manual lymph drainage
  3. functional activities
  4. meticulous skin care
  5. contraindications to modalities
  6. compression garments (at end of phase 1)
  7. Red flags
    1. strenous actvities

Q

Manual Lymph Drainage (MLD)

A
  • massage and PROM to assist lymphatic flow
  • emphasis on decongesting proximal segments first (trunk quadrant) then extremities, always distal to proximal
  • Compression using layered padding
    • bandages maintain limb after techniques
    • decongestive exercises

Q

Contraindications to Functional Activities w. Lymphedema

A

Red Flags:

strenuous activities

jogging and ballistic movements

high pressures will occlude superficial lymph

no modalities, or electric therapy >30 Hz

SIgns of Lymph Overload

discomfort, aching or pain in proximal lymph areas, change in skin color

Q

Lymphedema Management Phase 2

A
  1. skin care
  2. compression garments
  3. exercise
  4. lymphedema bandaging at night
  5. MLD as needed
  6. compression pumps: use with caution
  7. Red Flags
    1. pressures >45 mmHg are contraindicated as they cause lymphatic collapse
