Lumbar Spinal Stenosis (2) Flashcards
Lumbar spinal stenosis
Bilateral or unilateral back, buttock, or lower extremity discomfort, pain, or weakness
What is lumbar spinal stenosis aggravated by?
Walking and prolonged standing
What is lumbar spinal stenosis relieved by?
Stopping forward and sitting
What percentage of people with lumbar spinal stenosis are asymptomatic?
80%
What type of pain can lumbar spinal stenosis result in?
Both somatic and radicular pain
Are nerve roots always mechanically sensitive?
No they are mechanically tolerate
What makes nerve roots mechanically intolerate?
Becoming chemically sensitized
What factors increase likelihood of developing symptoms?
Severity of ischemia
Tissue health
Systemic inflammation (immune system)
Is lumbar spinal stenosis insidious or sudden pain?
Insidious
Claudication
Pain in the legs and lumbar spine
Vascular claudication
Arteries are not getting blood to muscles in the legs
Neurogenic claudication
Compressing nerves in central canal
What is a two staged treadmill test used to test?
Neurogenic claudication
What is the ankle brachial index used to test?
Vascular claudication
What indicates vascular claudication on an ankle brachial index?
<.9 (peripheral vascular disease)
What does flexion cause in claudication?
Less crowding so symptoms do not come on as quick
What is the difference between surgical and conservative care for lumbar spinal stenosis?
Same outcomes but conservative care does not have risks that surgery does
What are the treatment goals of lumbar spinal stenosis?
Improve blood flow to lumbar region
Increase local tissue health (deadlift)
Decrease systemic inflammation (sleep, nutrition, lifestyle changes)
