Hip Soft Tissue Disorder (2) Flashcards
Hip bursitis
Ischial- direct trauma or movement in sitting position
Iliopectineal- anterior hip pain
Greater trochanteric pain syndrome
Mimics L5 condition
Usually 40-60 year olds
2/3 have OA or LBP
Who is more prevalent for snapping hip syndrome?
15-40 year old
women>mem
What occurs in snapping hip syndrome?
Anterior groin pain with resisted hip flexion
Iliopsoas tendon tender to palpate
What occurs in gluteal muscle pain?
Dull lateral hip pain
Weak hip abduction
What is the conservative care approach for greater trochanteric pain syndrome?
Stretch Iliopsoas
Strengthen hip abductors
What is the conservative care approach for snapping hip syndrome?
Address muscle imbalances (stretch and strengthen)
What is the conservative care approach for gluteal muscle pain?
Decrease load
Motor control training
What are external causes of snapping hip syndrome?
Posterior iliotibial band
Anterior gluteus Maximus
Trochanteric bursitis
What are internal causes of snapping hip syndrome?
Iliopsoas tendon snapping
Iliofemoral ligament snapping
Hamstring syndrome
Ilipsoas capsular thickening
What are Intra-articular causes of snapping hip syndrome?
Labral tears
Loose bodies
Capsular instability
Displaced fractures
Synovial chondromatosis
What is the main treatment for snapping hip syndrome?
Arthroscopic release (80%)
What is the imaging in snapping hip syndrome?
Ultrasoudn
Which muscles of the hip are analogous to rotator cuff muscles of shoulder?
Gluteal tendons
Obturator nerve (L2-4)
Through psoas into obturator foramen
Femoral nerve (L2-4)
Exits lateral psoas and courses inferior between psoas and iliacus
Lateral femoral cutaneous nerve (L2-4)
Emerges from lateral border of psoas major, passes under inguinal ligament and then goes over Sartorius
How can the Piriformis nerve be injured?
Piriformis hypertrophy
Spasm or muscular fibrosis following trauma
Prolonged compression
How can the obturator nerve be injured?
Pregnancy
Trauma
Hip surgery
How can the femoral nerve be injured?
Trauma
Tumors
Surgery
How can the lateral femoral cutaneous nerve be injured?
Obesity
Tight belts
Repetitive hip flexion
How does a hamstring strain occur?
Quick explosive contraction (non traumatic)
Grade 1 hamstring injury
No time lost
Grade 2 hamstring injury
5-12 days
