Amides or Esters-Longer half life and longer duration of action
Amides. Becasue metabolized in liver
Ester metabolism
Rapid metabolism by hydrolysis in plasma via ChE and excretion in urine
Short plasma half life (less than 1 min) leads to short duration of action
Fiber size vs. minimum anesthetic concentration
Big fiber = big Cm
pH vs Cm
High pH=low Cm
Ca concentration vs Cm
High Ca = High Cm
factors affecting reversal of local anesthesia
Absorption into circulation (most important)
Dilution by ECF
Redistribution to other areas
Use of vasoconstrictors
Lidocaine
Used topically and as an injectable. Most common for epidural going to c section when total motor block desired
Dental anesthetic, not NO
Articaine
Benzocaine
Topical only anesthetic
