Dose dependent (zero order). Can be saturated.
Phenytoin mech of action
- blocks and prolongs the inactivated state of voltage gate Na channels
- enhances release of GABA
- Prevents seizure propagation
Phenytoin side effects
Gingival hyperplasia is the big one

Carbamazepine mech of action
similar to phenyotoin-inhibition of voltage gated Na channels
Carbamazepine metabolism
Potent inducer of P450s and its own metabolism
Oxacarbazepine
Newer, similar to carbamazepine. Shortr half life, active metabolite has longer duration and FEWER drug interactions
Carbamazepine clinical use
- General clonic tonic seizures
- Partial seizures
- Trigeminal neuralgia-drug of choice
- Manic phase of bipolar disorder
Carbamazepine side effect
SIADH-Syndrome of Inappropriate ADH secretion.
Teratogenic-cleft palate and spina bifida
Phenobarbital mech of action
Enhances phasic GABA receptor responses-increases opening time of Cl channel
Phenobarbital clinical use
- Neonatal seizures
