What is obstetric cholestasis?
Cholestasis that develops in the third trimester and usually resolves after birth, stoppage or reduction in bile flow
How does obstetric cholestasis present?
Generalised itching worse on hands and feet, no rash, RUQ pain, malaise, steatorrhea
What would you see on LFTs
in OC
Raised transaminases (ALT/AST), raised bile salts
What does OC increase risk of in the mother?
PPH due to vit K deficiency
What does OC increase risk of in baby?
?Preterm labour, still birth, meconium
how is OC treated
Prophylactic vit K- antihistamines for itch, ursodeoxycholic acid- itch
How is OC monitored?
Weekly LFTs and clotting screen- 2 wk USS and CTG
When should mums w OC deliver?
37-38wks
What is DD ofOC?
Gallstones, Acute viral hepatitis, primary biliary cirrhosis
