Liver 1 Flashcards
Role of:
1) ferroportin
2) hepcidin
3 hemojuvelin
1) exports iron across the basolateral membrane
2) Negative regulator of iron absorption (down regulates ferroportin)
3) protein involved in making hepcidin
Hereditary hemochromatosis classifications;
Type I
Type II
Type III
Type IV
1) HFE
2) Juvenile hemachromatosis (hemojuvelin; hepcidin)
3) transferrin-2 receptor
4) Ferroportin (Gain of function and loss of function)
when to send an HFE mutation analysis?
Went to obtain liver biopsy?
1) Elevated ferritin (200+ in women, 300+ in men) with no other Inflammation
2) Transferrin saturation over 45%
1) Ferritin >1000 or elevated LFTs In a patient with confirmed hemachromatosis
Treatment for hemochromatosis:
When to do phlebotomy? How much? Until?
Ferritin >1000 or elevated LFTs;
One unit a week;
HCT <35% and ferritin 20-50
Phlebotomy (in Hemochromatosis) will not fix the following once they’ve occurred
Cirrhosis, arthropathy, diabetes, testicular atrophy
Anyone with hemachromatosis should avoid this supplement? Why?
Vitamin C; increases iron absorption
