Lidocaine: Class
Antidysrhythmic
Sodium Channel Blocker
Lidocaine: Indications
- Supression of ventricular dysrhythmias (V-Tach, V-Fib, PVC’s)
- Prophylaxis against recurrance after conversion from V-Tach. VF.
Lidocaine: Contraindications
- 2nd & 3rd degree blocks in absence of a pacemaker.
- Bradycardia
- Stokes-Adams syndrome
Lidocaine: Adult Dose/Admin
Adult
Cardiac Arrest: 1-1.5mg/kg IV/IO bolus. May repeat after 3-5 min at 0.75mg/kg. Max total dose of 3mg/kg.
After converstion to NSR: Maintenance infusion at 2-4mg/min.
V-Tach w/ Pulse: 1-1.5 mg/kg IV/IO bolus. May repeat after 3-5 min at 0.5-0.75mg/kg. to max of 3mg/kg.
Begin maintenance infusion at 2-4mg/min ASAP.
Lidocaine: Pediatric Dose/Admin
Pediatric
1mg/kg IV/IO bolus.
Infusion: 20-50mcg/kg/min.
Lidocaine: For IO Access
Unstable patients able to percieve pain:
Adult
40mg over 2 min
Pediatric
1mg/kg to max of 20mg.
Both followed by 10ml NS slow bolus over 5sec.
