Comprehensive initial, focused, emergency, time-lapsed
Define ongoing assessment.
Problem-specific assessment that consists of continuous data collection
Define emergency assessment.
Performed during physiologic or psychological crisis
Define time-lapsed assessment.
Reassessment to compare pt’s current status with baseline status obtained earlier
Define objective data.
Observable, measurable data that can be seen, heard or felt by someone other than the person experiencing it
Define subjective data.
Information perceived only by the affected person
What are the four steps of data interpretation and analysis?
Recognizing significant data, recognizing patterns or clusters, identifying strengths and problems, reaching a conclusion
What is the main goal of concept mapping?
To establish relationships between concepts

Encourages pt to elaborate on thoughts and feelings

Validate or clarify information
Define nursing diagnosis.
Defines pt problems that nurses can treat independently

Describes problems for which physicians direct the primary treatment
