Lecture 8 Drop instillation Flashcards
How is absopriton to the nasopharyngeal mucosa minimized with drop instillation?
punctal occlusion
- Using 10% phenylephrine to avoid hypertensive crisis
- Beta blockers to avoid breathing problems and bradycardia
- Adrenergic agents to avoid tachycardia and hypertensive crisis
- -all of these describe when a ______ occlusion is essential
punctal
you use punctal occlusion when using 10% phenlephrine to avoid (hypotension/hypertension) crisis
hypertension
you use punctal occlusion when using beta blockers to avoid ___ problems and bradycardia
breathing
you use punctal occlusion when using adrenergic agents to avoid (bicardia/tachycardia) and hypertensive crisis
tachycardia
Do you need the pt consent when putting in drops? do you need to note it in chart?
yes, yes
What four things should you tell the patient before drop intillation
- plan on instilling drops
- may sting
- normal aciton of the drops and how long the effect will last
- potential side effect of the drops
- —dont forget to check expiration date! (can let pt know )
T/F before putting in drops you must check that there are no allergies, drug interaciton, systemic conditions, ocular anatomical conditions that will interfere with the drops or have unwanted side effects
T
T/F it is okay for the pt to take the tissu and dab thier puncta after you have done the drop instillation
F . Ask them to only wipe the excess drops and not drain their eyes of the medication with the tissue
Which hand should the bottle cap be in?
non-dominant hand
describe the two handed technique for drop instillation
Left hand holds upper lid, right hand pulls down lower lid and instills drop
T/F Hold the dropper far away enough from the patient’s eye so that an unexpected blink will not contaminate the bottle
T
T/F You should never ask the pt to occlude the puncta himself
F you may ask pt to occlude the puncta by straddling the bridge
About how long should the pt keep their eye closed for after instillation?
3 minutes
Why is it important to keep eyes closed for 3 minutes after intillation?
This will keep the amount of drug pumped into the naso-lacrimal system by the lids and orbicularis muscle to a minimum
