Precautions of neural glides
increased irritability of tissue with treatment
watch for symptoms of vascular compromise; pallor, cool skin
Contraindications of neural glides
acute or unstable neurological signs
cauda equina symptoms = changes in bowel or bladder
spinal cord injury
neoplasm and infection
active disease or pathology in nervous system
Sliding of Median nerve
start with elbow flexion and wrist flexion, neck is bent to opposite side
when pain begins, elbow should extend, wrist extend, neck bends towards arm
progress up to 90°
Sliding of nerve
movement of 2 or more joints in chain in which 1 movement counteracts the other. use when highly irritable
10 reps, slowly, 2-4 times a day
Tensioning of nerve
movement of 1 joint in chain to elongate nerve bed. use when tissue is less irritable
10 reps, slowly, 2-4 times a day
Testing for median nerve
used for exam and treatment of symptoms
shoulder depression/elbow wrist are flexed
shoulder abduction 110°
ER
elbow extension
forearm supination
wrist and finger extension
Tensioning of median nerve
Elbow and wrist flexed, neck in middle
move to elbow/wrist extended, neck to opposite side
increase to 90° of abduction
Neural testing of sciatic
Supine
Left leg with knee in extension
dorsiflex ankle
hip adduction
IR
dorsiflex w/eversion
tibial nerve (TED)
dorsiflex w/inversion
sural nerve (SID)
plantarflex w/inversion
common peroneal (PIP)
Sliding for sciatic
Dorsiflex ankle, head back. Plantar flex toes, move head down when pain begins
arms behind back, can be sitting or standing
Tensioning for sciatic
laying on back
leg straight
bend knee back and forth up to the point of tension
Positive symptoms of nerve pain
abnormal level of excitability in nervous system. pain, paresthesia, dysesthesia, spasm
Negative symptoms of nerve pain
reduced impulse conduction in the neural tissues. hypoesthesia or anesthesia, weakness
