When do maternal mental health issues present?
antenatal (during pregnancy), postpartum (birth and the following year)
Why might mental health issues present during pregnancy?
women may cease exisiting medications, pregnancy can be stressful, substance use during pregnancy
What is the biggest concern for effect on the foetus due to maternal mental health issues during pregnancy?
Substance abuse
What are the 3 stages of the spectrum of postpartum mood changes?
transient/nonpathologic, serious/disabling, psychiatric emergency
What are the ‘Baby Blues’
On the 3rd to 5th day after birth, feelings of being overwhelmed, tearful, exhausted, etc for several hours of days.
If the ‘Baby Blues’ persist beyond 2 weeks psot-partum what should the mother be screened for?
Post-partum depression
Within what timeframe must the onset of depression be to be considered PPD?
Within 1 month after birth
What is the prevalence of Postpartum Depression?
10-16%
Is the DSM-5 criteria for Major Depressive Disorder and Postpartum Depression the same or different?
The same - PDD criteria the same as a woman in any other stage of life.
What are the risk factors for PPD?
1.) previous history of depression
2.) discontinuation of medications
3.) childhood abuse
4.) negative attitude to pregnancy
5.) lack of support
6.) multiple birth
7.) baby loss
8.) pregnancy or birth complications
What are the possible biological factors in PPD?
sudden change in hormone levels, lack of sleep, pain, feeding issues
How is PPD typically detected?
Screening of all women after birth: consultation with patient/family, Edinburgh Postnatal Depression scale which is self-administered
What is the effect of PPD on mother-infant interaction?
decrease in affection, decreased responsiveness to infant cues
What are the effects of untreated Postpartum Depression on childhood development?
Behavioural problems, poor educational outcomes, poor social competences, risk of mental disorder and risky behaviour
What is the key risk factor for Postpartum Psychosis?
History of bipolar disorder or schizophrenia
