low exposure, enzyme induction of inactive metabolites in liver, and impaired 1alpha hyroxylase in kidney

high PTD but also high receptor resistance, short 4th and 5th metacarpal, mutation of Gs

Autosomal dominant hypercalciuric hypocalcaemia?
A
CaSR activation, parathyroid reading serum Ca as higher than truth, PTH secretion becomes inappropriately low

Familial Hypocalciuric Hypocalcemia?
A
normal PTH, family history of hypocalcemia, rato of fasting urine calcium to creatinine clearance
