Q

How are primary bile acids synthesized?

A

synthesized in liver from cholesterol

rate limiting enzyme = 7 alpha hydroxylase

6
Q

How are secondary bile acids formred?

A
  • conjugation of primary bile acid with AA glycine or taurine
  • secreted in bile
  • oxidized by intestinal bacteria

7
Q

What happens to bile salts in intestine?

A

70-80% reabsorbed in distal ileum and return to liver via portal vein

10-15% reabsorbed in colon after bacterial deconjucation

10-15% lost in feces

Q

What percent of bile salts in circulation are recirculated vs new?

A

> 95% recirculated

Q

What are some of bile duct disease?

A

things get backed up; have cholestasis = bile standing still

  • -> excess bile acids –> hepatocyte injury
  • -> excess bilirubin –> jaundice
  • -> excess cholesterol –> xanthomata

16
Q

When do you get pigmented gallstones?

A

when high concentrations of unconjugated bilirubin in bile

due to hemolysis, liver disease, infected bile, starvation of total parenteral nutrition

17
Q

What are pigmented gallstones made of?

A

bilirubin and calcium = not crystals

18
Q

What is relationship acidity of gallbladder and likelihood of stone precipitation?

A

more acidic –> less likely to get stones

19
Q

When do you get cholesterol gallstones?

A

40, female, fat, fertile

20
Q

Where do cholesterol vs pigmented stones localize?

A

cholesterol stone = in gallbladder not ducts

pigmented = can arise in either gallbladder of bile ducts

21
Q

What is biliary colic?

A

stone stuck in cystic duct causes pain

22
Q

Are you more likely to get gallstone in fed or fasting state?

A

fasting

23
Q

What is acute cholecystitis?

A

cystic duct obstruction + infection

80% subside w/ antibiotics alone

24
Q

What can you give to dissolve cholesterol gallstones?

A

ursodeoxycholic acid = main constituent of bear bile; liquefies the bile

side effect = diarrhea

25
Q

What is biliary sludge?

A

history looks like biliary colic but may not have an actual stone yet
