Q

why is it essential that the different professionals involved in an individuals care communicate with eachother effectively and work as a team?

A

to reduce the chance of horrendous consequences happening due to a lack of ‘joined up working’ such as in the case of baby P and victoria climbie. in both cases the workers did not pass on crucial information, resulting in the death of the children which could have been prevented
