Albumin: major plasma protein- oncotic pressure and protein transport
- Slow to fall, normal in acute hep or toxicity.
- Reduced in chronic cirrhosis, malnutrition, protein loss or increased blood volume
- Increased in dehydration, anabolic steroids
Alkaline phosphatase
- Elevated in bone disorders, hepatocelluar disease, pregnancy & cholestatic syndrome
- Lowered in hypothyroid, phosphatemia, anemia, Mg deficiency
Gamma- Glutamyl Transpeptidase
- Biliary excretory enzyme
- Used to determine if ALP is from hepatic etiology
- Elevated in alcoholic liver, cholestatic disorders
- Not elevated in bone disorders
Bilirubin- Direct
- Excreted in the urine
- Increased in choestatic liver disease- bile
Bilirubin- Indirect
- Increased with break down of erythrocytes: hemolysis, gilbert disease- not serious liver disease
AST
Marker of inflammation & injury, released from liver. Less specific for liver than ALT- released from muscle, cardiac, brain
ALT
Found predominantly in liver. Indication of inflammation and injury- found within cells and released with death.
Amylase
Secreted by pancreas- break down starch in to glucose
- Increased in acute or chronic pancreatitis
Lipase
Secreted by pancreas, fat digestion
- Indication of pancreatitis
- Longer 1/2 life than amylase
Lactate dehydrogenase
Increased in alcoholic liver, carcinoma, pancreatitis, obstructive jaundice & hepatitis
Total protein
- Albuimin & globulin
- Marker of edema & nutritional status
- Increased in chronic liver disease
SAAG gradient
- Determine cause of ascites- calculation of ascitic albumin - serum albumin
- Increased in cirrhosis, hepatitis, budd-chari, liver diseases
- Decreased in pancreatitis and peritoneal issues
