Lab 7 abdominal sx Flashcards
First structure encountered in abdomen is
Cecum
Cecum lies ventrally on
Right side
Apex of cecum directed
cranially
How many bands does the cecum have?
4
The lateral band of the cecum becomes
cecocolic fold, leading to right ventral colon
Which colons are sacculated?
Ventral colons
How many bands on small colon
two bands, one within mesentary
Proximal colon attached to duodenum by
duodenocolic ligament
Dorsal band of the cecum runs into
ileocolic fold, which continues onto antimesenteric border of the ileum
The ileum is positively identified by
antimesenteric fold, which is continuous with ileocecal fold
Junction of the proximal jejunum and distal duodenum identified by
duodenocolic ligament
-arises on antimesenteric side of duodenum and joins the small colon
Duodenum lies in a gutter between
right dorsal colon and right dorsal body wall
Epiploic foramen found by
grasping duodenum between fingers and thumb with dorsal surface of fingers against caudate lobe of liver
- opening found by moving right to left
- located just cranial to right kidney
The stomach and the spleen are located in the
left quadrant
Nephrosplenic ligament is
from left kidney to dorsal tip of spleen
In a stallion, internal inguinal rings are located
ventrolateral to pelvic brim
Complications of performing a jejuno-cecostomy
- bypass of iliocecal valve
2. creation of sharp transition between intestinal segments of dissimilar functions
Purpose of Parker Kerr technique
- Minimal contamination
- Excellent serosal apposition
- Watertight seal
Abdominal closure
- Linea closed in simple continuous pattern with #2 vicryl
- SC closed with simple continues
- Skin closed with interrupted or continuous pattern
Type of suture for stay sutures
2-0
-don’t tie these
Closure of enterotomy
- Use 2-0 suture, start pattern away from dominant hand (right to left) but direct needle towards yourself in simple continuous pattern
- bites 8 mm to 1 cm apart
- start and end in intact bowel
- Cushing layer start with first bite going in opposite direction to remainder of suture pattern so knot is placed in the suture line, not away from it.
