Lab 11/12 - Shoulder Flashcards
When observing the shoulder what should you look for
- Shoulder, scapular and neck positioning
- Any asymmetries
When palpating what bony landmarks should you look for in the shoulder
- Scapula (borders)
- Acromion
- Clavicle
- Coracoid Process
- Greater/Lesser Tuberosity
- Bicipital Groove
When palpating , what soft tissues in the shoulder should you look for
- Latissimus Dorsi
- Rhomboid Minor and Major
- Teres Minor and Major
- Upper, Mid and Lower traps
- Deltoid
- Supraspinatus
- Infraspinatus
- Levator Scapulae (Scalenes)
When testing active and passive range of motion what should you be looking for?
- Scapular elevation, depression, protraction, retraction, downward and upward rotation
- Shoulder flexion, extension, abduction, adduction, internal and external rotation
When measuring PROM and AROM , what is the difference with using the goniometer and movement
PROM: Move goniometer at same time you are moving patients arm
AROM: Can move goniometer after patient moves own arm
T or F: You want to align the goniometer with the joint
T
How to observe a patients AROM of scapula
On initial assessment observe scapula and back and look for:
1) Height difference between 2 shoulders (dominant hand should be slightly lower
2) Get to upward and downward rotate while observing scapula
3) Have reach on both sides by getting them to touch their back to observe differences between each arm
4. Have protract and retract
5. Have elevate and depress
Go through Scapula gravity postions
Sitting for elevation, depression, protraction, retraction, downward rotation and upward rotation
Go through scapula anti gravity positions
Side lying for elevation, depression, protraction, retraction, downward rotation and upward rotation
Go through shoulder joint anti gravity positioning
Flexion/extension -> side lying standing behind back
Abduction/Adduction -> Lying supine (bring rotating upwards/downwards against bed
External/Internal Rotation -> Sitting elbows to side @90 degrees and move outwards and inwards
Horizontal abduction/adduction -> bring arm across chest and back out lying supine
Go through shoulder joint gravity positioning
Flexion/extension -> sitting
Abduction/adduction -> sitting
External/internal rotation -> sitting elbow and arm bent at 90 degrees and rotate forearm upwards and downwards _|
How to apply scapula strength testing (make test)
Elevation-> Push down on shoulder
Depression -> laying prone arm extended above head and apply pressure down on arm
retraction -> push against arm while they retract laying prone
protraction -> sitting up and push against while they protract
Upward Rotation -> push against while sitting
downward rotation -> laying prone, arm behind touching lower back and pull back on arm while they pull down
How to apply shoulder joint strength testing (make test)
Same as shoulder gravity and anti gravity positioning but this time you are applying pressure
