Spasticity is present due to resistance to passive movement, no voluntary movement is present a stimulus can elicit limb synergies reflexly (flexion synergy dominates arm, and extension synergy dominates leg)

A

Spasticity is marked and synergistic movement can be elicited voluntary, but are obligatory (flexion synergy dominates arm, and extension synergy dominates leg)

A

Spasticity decreases and synergy patterns can be reversed if movement takes place in the weaker synergy first. Movements combining antagonistic synergies can be performed if prime movers are strong parts of synergies.

Stage 5

A

Spasticity wanes but is evident with rapid movement and extreme range. Synergy movements can be reversed. Movements become environmentally specific and weak components of synergies start to be performed.

A

Coordination and pattern of movements are near normal and spasticity is no longer present. Environmental patterns are now possible, but abnormal movements emerge when rapid or complex movements are performed.

What is the standard starting position for the arm assessment

A

Sitting with forearm in lap or supported on pillow in lap in neutral position, 0 degrees at wrist and fingers slightly flexed. Sitting unsupported over side of bed or supported in chair/wheel chair. Feet supported and good posture.
