Bowel/bladder disturbance or other sign of spinal cord compression
Incapacitating nerve root pain despite 4 week conservative TX

associated with gastrocnemius and soleus
Sensory - plantar/lateral foot
Motor - plantarflexion, toe flexors, hip extension
Pain - bottom of foot, posterior calf/thigh, buttocks
