Higher frequency of firing

Nerve root lesion
weakness of multiple muscles with same root of innervation
Terminal branch lesion
weakness of one nerve distribution (even multiple roots)
Plexus lesion
weakness include multiple nerve roots and multiple nerves
Peripheral neuropathy
weakness if stocking/glove or all over
Myopathy
weakness is proximal distribution, bilateral wiht sparing of distal muscle
L2 L3 L4 L5 S1
L2 - hip flexion L3 - knee extension L4 - ankle dorsiflexion L5 - big toe extension S1 - ankle plantarflexion
UMN signs
Spastic paralysis Hyperreflexia Spasticity Clonus Babinsky/Hoffman Sensory loss
LMN signs
Flaccid paralysis Hyporeflexia Atrophy Sensory loss Fasciculations
Four muscles to test strength
Upper trapezius
Triceps
Quads
Gastroc
Strength scale
5 - full ROM/strength 4 - full ROM w/ less strength 3 - full ROM against gravity only 2 - full ROM w/out gravity 1 - no joint motion, slight contraction 0 - no joint motion or contraction
Reflex testing
0 - Areflexia 1 - hyporeflexia 2 - normal reflex 3 - hyperreflexia 4 - clonus
