What do symptoms of ITP depend on?
Platelet count
What are the main symptoms?
Purpura
Ecchymosis
What are purpura and ecchymosis?
Purpura = non-blanching spots Ecchymosis = bruising
Typical presentation
” 50 y/o woman presents with easy bruising and bleeding from the gums. O/e she has petechiae and bruising all over. Her liver and spleen are not palpable. FBC reveals thrombocytopenia with a count of 12 but all other cell lines are normal. “
Investigation?
FBC
- Only thrombocytopenia
Coombs test
- +ve
Bone marrow biopsy
What is seen on bone marrow biopsy?
Increased megakaryocytes
Mostly done to rule out malignancy
Initial treatment?
IVIG
Steroids
Can give platelet transfusion in severe thrombocytopenia but will just be broken down by immune cells
Surgical treatment?
Splenectomy
Non-severe bleed?
Observation
Steroids
