IOI-5 Refueling Flashcards
State the purpose of IOI-5 Refueling
to provide instructions for plant SHUTDOWN from IOI-3 to Mode 5 (REFUEL) and back to Mode 4
Who maintains the official SNM tracking sheet?
Reactor Engineering
only a copy in the control room
State the items personnel admitted access to the refueling floor must be familiar with
area rad monitors and evacuation procedure
state when continuous communication is required between the control room and the refueling platform
during CORE ALTERATIONS
State where travel with loads in excess of 1140 pounds is prohibited
prohibited from travel over fuel assemblies in the spent fuel or upper ctmt fuel storage pool racks
under what conditions are personnel required to leave the reactor “line-of sight”?
RP personnel may order any personnel out of the line of sight area should dose levels warrant
identify the restrictions on loading fuel into the reactor vessel with regard to control rod position
fuel loading into the reactor core is prohibited with any control rod withdrawn in order to prevent the potential for criticality excursions except reload per an approved spiral reload sequence is permitted WHEN rods are withdrawn/removed per LCO
what actions are required if forced circulation or shutdown cooling is lost?
to prevent temperature stratification in the RPV, the reactor recirc pumps should be maintained operating in slow speed. refer to att and onep if forced circulation or shutdown cooling is lost
who must be notified before re-aligning the FPCC system, swapping RHR/ADHR SDC, or starting any other injection systems (i.e., condensate, feedwater, recirculation, etc.)?
the control room AND refuel floor must coordinate
what actions are required if reactor coolant or fuel pool temperatures drop below 68F?
All fuel movement activities must be suspended until additional analyses are performed or the temperature is restored above 68F
what conditions would require operators to initiate action to restore secondary ctmt with one standby gas treatment system in operation?
NO shutdown cooling available for extended time, before exceeding time to 200F RX coolant temp
OR Reactor/Refuel cavity drain-down without makeup capacity
OR control rod drop OR miscellaneous equipment drop on the fuel with indication of damaged fuel
who must observe core alterations?
SRO with no other concurrent duties. “licensed SRO” or “SRO limited to refuel” qualified
State two methods available to control the vessel cooldown rate in preparation for refueling
when the reactor water level reaches the flanges, a rise in flange cooldown rate is expected. adjusting vessel level will assist in controlling the cooldown rate to <90F/hr
Adjust Shutdown cooling flow to maintain reactor vessel pressure below the shutdown cooling high pressure isolation while flooding
state the number of SRVs that must remain functional until the reactor head is removed
2 SRVs
Opening of HFTS hatch when both CTMT air locks AND CTMT hatches are closed can cause what?
CTMT pressure to rise resulting in pool level control problems
