Interview itself
note taking (use abbreviations), subjective portion (open-ended questions first, then more focused), exam
Chief complaint
patient’s main reason for visiting the doctor - in patient’s own words
HPI
History of present illness - their story about how they’ve been feeling and why they are seeing you
OLDCAAARTS
pneumonic for details of HPI
Onset (when, what doing, ever happened before), Location (anatomic position of symptoms), Duration (constant vs come and go), Character (adjectives), Alleviating/Aggravating/Associated symptoms, Radiation (same pain vs diff pain), Timing (throughout day), Severity (rate 0-10, how much does it affect their daily life)
Associated symptoms
Anything else going on that may be related to the chief complaint. Tends to be in the same body system. (N/V/D)
Review of Symptoms
find out other issues the patient may have, and can guide to see associated symptoms for HPI.
** For exams - must ask at least 2 symptoms from 2 different systems, cover 3 symptoms minimum
Remaining subjective data
ROS
Previous Medical History
Previous Surgical History
Medications (Rx, OTC, Supplements) - dose and timing
Allergies (and reactions associated with)
Family history
Social History
