Commonly found in women, black patients, granulomatous in airways and lung parenchyma; honeycomb CXR
Sarcoidosis
Common presentation of SX in Sarcoidosis
Lofgren syndrome (swelling, arthritis, fever); Lupus Perino
Treatment for Sarcoidosis
Spontaneous remission can happen; Treat with steroids if necessary
Necrotizing granulomas, arthralgias, anorexia, weight loss, nasal and sinus issues with bloody discharge; CT shows nodules and ground glass
Pulmonary Vasculitis (Granulomatosis with Polyangiitis also known as Wegener granulomatosis)
Presentation: adult onset asthma, loss of appetite, joint pain, GI bleed; Blood vessel inflammation that causes restricted blood flow and organ damage; unknown cause
Eosinophilic granulomatosis with polyangiitis (Churg-Strauss syndrome):
Glomerulonephritis, pulmonary hemorrhage, and anti-GBM antibodies
Alveolar Hemorrhage Syndrome (Goodpasture syndrome)
Treatment for Chaug-Strauss and Goodpasture syndrome
Corticosteroids and immunosuppressants
Presentation: Rapid renal insufficiency, cough, hemoptysis; CXR shows pulmonary infiltrates and Ground Glass; Glomerulonephritis seen in biopsy; Labs show blood and protein in urine
Alveolar Hemorrhage Syndrome (Goodpasture syndrome):
Present with dyspnea, upper lobe nodular opacities, and “egg shell” calcifications on CXR; works in rock quarry or stone cutting
Silocosis
Chronic, productive cough; CXR shows pigmented lesions surrounded by emphysema
Coal workers’ Pneumoconiosis:
Very common; abnormal cellular and humoral response from organically inhaled antigens; CXR shows non-specific infiltrates and ground glass opacities; present with cough and dyspnea hours after exposure
Hypersensitivity pneumonitis
Treatment for hypersensitivity pneumonitis
Corticosteroids, and antifibrotics
