Pathophysiology
A
group of conditions that affect the interstitium (lung tissue between the alveolar epithelium and the capillary endothelium) of the lungs
excessive healing - inflammatory response to injury leads to fibrosis and interstitial thickening
possible destruction of the capillary bed
resultant pulmonary HTN right sided heart failure
deconditioning results in peripheral muscle dysfunction

Clinical Implications
A
reduced lung volumes impaired gas exchange airflow limitation dyspnoea reduced exercise tolerance musculoskeletal dysfunction

Clinical Features
A
dyspnoea with exertion dry, persistent cough fine inspiratory crackles on auscultation possible finger clubbing spirometry - has restrictive pattern very low SpO2

Medical Management
A
supportive care (not much evidence for inhaled therapies)
- pulmonary rehab
- O2 therapy
- lung transplant

Physio Management
A
- pulmonary rehab
- optimise conditions for transplant
- NIV - most likely patients won’t be able to handle the back pressure
- assessment to guide medical management
- management of breathlessness
