Method of insertion
A
- Consent
- Sterile
- Pt placed supine
- Arm above head
- 4/5th ICS, ant. axillary line
- Local anesthetic (skin, subcut, chest wall)
- 2cm incision above rib
- Blunt dissection down
- “Pop” pleura
- Insert finger to feel for adhesions
- Insert clamped tube to level 15
- Close incision with mattress stitch
- Secure with elastoplast
- Attach to drain
- Unclamp - check if swinging

A
- Pneumothorax
- Haemothorax
- Chylothorax
- Symptomatic pleural effusion
- Empyema

Prerequisites to remove ICD
A
- Drains less than 50ml/8hrs, or 150ml/24hrs AND still swinging
- Repeat CXR shows full expansion
- Indication resolution

- Base - 5th ICS
- Post. border - Lattimus dorsi
- Ant. border - Pectoralis major
- Apex - Axilla

Maximum Lignocaine dosage
A
- With adrenaline: 7mg/kg
- Without adrenaline: 5mg/kg
