Elements of informed consent?

what are the exceptions of informed consent?
minor procedures, emergencies (unconscious?), therapeutic privilage, waived right to be informed, obvious risk, public health requirment,
who may give consent?
competent patients, surrogate decision maker (poa), spouse/relative, living will
what are some of the special circumstances regarding informed consent?
need special consent for use of restraints, parents of minors not informed re: stds,etc, mental health pats cant be held against will, research studies need written consent, prisoners 5th amendment
