Inflammatory disorders 2/ Autoimmune systemic disorders Flashcards
Autoimmune Dermatological condition
Psoriasis
Autoimmune Neurological condition
Multiple sclerosis
Autoimmine systemic disorders w/ Msck system involvement
- RA
- AS
- Reactive arthritis
- Reiter’s syndrome
symptoms pf Psoriasis
1) dermatological –>** thick, silvery scales** and itchy, dry red patches that are sometimes painful
2) Psoriasis patches can range from a few spots of dandruff-like scaling to major eruptions that cover large areas
Symptoms of Rheumatoid arthritis
1) Chronic arthritis
2) joint pain
3) morning stiffness
4) limited range of motions of affected joints
5) Symmetric involvement of the small joints, hands and feet
Extraarticular manifestations of Rheumatoid arhthritis
Inflammation sometimes can affect other organs beside the joints for instance, the eyes or lungs
classification of RA
seropositive : positive Rheumatoid factor
seronegative : negative Rheumatoid factor
**
Serological investigations in RA
ANA , RF, antiCCP
———–: Inflammatory disease that, over time, causes fusion of the spinal cord resulting to rigid and less flexible spine. Long term resulting in a hunched-forward posture
Ankylosing spodylitis
CF of Ankylosing Spondylitis
1) pain
2) morning stiffness (lower back and hips),
3) neck pain and fatigue also are common
symptoms might worsen, improve or stop at irregular intervals
Areas most affected in Ankylosing spondylitis
1) sacroiliac joints , usually bilateral
2) enthesitis, where tendons and ligaments attach to bones
3) tendinitis
Serological tests in Ankylosing Spondylitis
HLA b27
—— : a painful form of inflammatory arthritis, It occurs in reaction to an infection by certain bacteria (Chlamydia)
Reactive arthritis
**
Bacteria that cause Reactive arthritis
1) Chlamydia Tachomatis
2) Salmonella
3) shigella
4) Yersinia
5) Camplylobacter
Symptoms of Reactive Arthritis
1) swelling and pain of heels, toes, fingers, low back, and joints, especially knees /ankles
2) extensive swelling of the fingers and toes
3) unilateral - not often symmetric
Epi of Reactive arhtritis
Men btw 20-50 yrs
Serological tests for Reactive arthritis
positive for
salmonella, Shigella, Yersinina, Campylobacter antibodies
Symptoms of Reiter syndrome
1) arthritis often in knees, ankles, and feet
2) conjunctivitis
3) urethritis
self limited between 2 to 6 months
Epi Polymyalgia Rheumatica
W>M
* 70 yrs old
* Caucasians
* inflammatory disroder
15 percent of people with PMR develop a potentially dangerous condition called ———
* PMR : polymyalgia rheumatica
giant cell arteritis (also known as temporal arteritis)
CF of Polymalgia Rheumatica
pain and stiffness in at least 2 of the following areas
1) Buttocks
2) neck
3) hip
4) thigs
5) Upper arms and shoulders
- systemic manifestation: fever, weightloss, poor appetite, fatigues
Blood tests for the diagnosis of Polymyalgia Rheumatica
1) ANA
2) Anti-CPP
3) complete blood count- CBC
4) CRP
5) ESR
6) RF
Antiphospholipid syndrome CF
2) DVT, PE
3) Transitory ischemic attack (TIA)
4) Repeated miscarriages/ still birth
——-: syndrome where the immune system attacks some of the normal proteins in the blood. It can cause blood clots in both arteries or veins
Antiphospholipid syndrome
Diagnosis of Antiphospholipid syndrome
ESR, CRP, b2 glycoprotein, Lupus anticoagulant, antiphospholipid antibodie
—–: systemic connective tissue disease
Systemic sclerosis
CF of Systemic sclerosis?
1) Raynauds
2) digital ulcers
3) dysphagia
4) telengiectasias
Diagnosis of Systemic sclerosis
ESR, CRP, ANA, ENA - special focus on the anti-centromere antibodies, RNP
**———- **a rare autoimmune disorder featuring signs and symptoms of three different disorders: lupus, scleroderma and polymyositis
Mixed connective tissue disease (MCTD)
Mixed connetive tissue disease is a rare autoimmune disorder featuring signs and symptoms of three different disorders: ——–, ———–, ———
Lupus, Scleroderma and Poylmyositis
Symptoms of mixed connective tissue disease
1) systemic sx: fatigue, mild fever,
2) Raynauds,
3) swelling that gives fingers a sausage-like appearance and
4) muscle and joint pain.
* In the later stages of the disease, the heart, lungs, kidney or other organs
may be affected
Diagnosis of Mixed connective tissue disease
RNP - POSITIVE
——-: inflammation of the muscles
Myositis
Types of Myositis
1) polymyositis
2) Juvenile dermatomyositis
3) Dermatomyositis
* juvenile form is the only type w/o symetric muscle weakness
Juvenile dermatomyositis differs from the adult form because of the coexistence of ——–
Vasculitis
Symptoms of Dermatomyositis
1) muscle weakness
2) muscle pain
3) fatigue
4) difficulty moving limbs
5) difficulty swallowing or breathing
Chracterized by patchy purple rashes in joints of the knuckles, elbows, knees and toes, they can also appear on the face, chest and back
CF of Polymyositis
1) muscle weakness especially in the thighs, hips and shoulders
2) lung issues, joint pain, fever, weight loss, Raynaud’s phenomenon and a general ill feeling
